CARE MANAGEMENT FOR AT RISK CHILDREN PROGRAM

The CMARC program is an early childhood program that focuses on improving health care outcomes for young children. Care Managers coordinate services with Medical Providers to ensure appropriate health and medical care, developmental and social supports are obtained to increase the quality of care provided and decrease the cost.

PREGNANCY CARE MANAGEMENT PROGRAM

PREGNANCY CARE MANAGEMENT PROGRAM

The Pregnancy Care Management (PCM) Program is designed to improve birth outcomes, increase full-term births and birth weights, and decrease C-sections, as well as improve the quality of care given to pregnant Medicaid recipients.

Diabetes Education Program
BRUNSWICK COUNTY DIABETES EDUCATION PROGRAM
Brunswick County Health Services provides an ADA recognized Diabetes Education Program to people who have Type I or Type II diabetes.  Referrals are accepted from your health care provider.  To learn more about the program, please view the brochure. Medical Nutrition Therapy

Brunswick County Health Services offers Medical Nutrition Therapy(MNT)by a registered dietitian nutritionist (RDN). MNT is a key component of our Diabetes Management Program. Nutrition-based treatment provided by the RDN includes a nutritional diagnosis as well as therapeutic and counseling services to help treat various medical conditions.

Health Education

The Health Education Division provides innovative educational programs that promote the physical, emotional, and social well being of individuals and communities. Health Education is responsible for planning, organizing, and implementing programs for clinic patients, community groups, civic organizations, private agencies, worksites, and schools. Other services include developing and marketing health promotion and risk reduction activities; consulting with other health professionals; and collaborating with other agencies to implement policy and environmental changes within the community.
