Forms

• Voter Registration Form (PDF) - An original form is needed for new registration and must be mailed or hand-delivered. If you are already registered and using the form to change information, the form may be emailed, faxed, mailed, or hand-delivered.

  • ONLINE OPTION:  If you are a DMV customer with an NC driver's license or DMV-issued ID, you may register to vote or change certain parts of your registration online. You may use this service to register, update your address, and update your party affiliation. You may not change your name.

• 2026 Absentee Ballot Request Form (PDF) - Registered voters can use the NC Absentee Request Form to request a ballot. The form cannot be returned electronically; it must be submitted by mail or in-person to the Board of Elections.

  • Registered voters can request their absentee ballot online through the NC Absentee Ballot Portal.

• Voter Registration Cancellation (PDF) - Use this form to cancel your Brunswick County voter registration.
• Notification of Deceased Voter Form (PDF) - Must be completed by the relative or personal representative of the deceased's estate.
